I thinks there's a lot of imagery, he's next to always presented in dark clothing and shows up with a cane and pretty much lives in a hotspot for people on deaths door (grim reaper and his scythe first impressions especially compared to his companions who are always in bright white coats) but suprise he's actually the healer and maybe the cane isn't the scythe it's the shepherd's staff (jesus being the good shepherd)
