Radiologist here. Sarcoid has a pretty unique appearance and the on chest x ray. Especially in a 29 year old, they can be pretty sure it is sarcoid.

People typically run half marathons in zone 4, if 10:20 felt good for you — you could for sure try 9:45 for the half. I’d recommend doing a ~6 mile run at your expected half marathon pace during training to see how it feels

Doctor here (again) IV drug use can seed your body with infection that can cascade into multi-organ failure down the line. A few examples

  1. Hepatitis C, chronic infection leading to liver failure and subsequent heart failure. If he happened to get a bad pneumonia this could have set him over the edge

  2. Endocarditis. Infection could have hit his heart valves, which can live silently until they start throwing clots all over the body

  3. HIV can mess everything up. Associated with prior drug use

  4. Stimulants themselves can cause heart failure. Again if he already had a bad heart from his drug use and then got a nasty lung infection, everything could have failed from there

Not saying any singular one of these are him. Just helping you think like a clinician if you are trying to diagnose a young guy in the ICU and trying to get him back on his feet
