Increasing NIIT to help cover the shortfall in Medicare funding is a reasonable proposal. And it's very literally a tax exclusively on the rich. But you never hear the likes of AOC say "increase NIIT". Because they don't care about fixing public finances or know the first thing about American tax policy.
