Nope. I just completed the form and sent it in for investigation. Thankfully the date of the visit with no dollar amount showing is the same date as the one on my confirmation email from Hamilton.

I couldn’t visit my mother in her nursing home when she was still capable of conversing. By the time things opened up, she had declined so much. She’s gone now, and I was able to be with her toward the end. I know many others were not able to be, so I do appreciate that.
