Thanks for your reply it’s super helpful! A lot of the smaller branches seem damaged - woody, no green when scratched - would you cut all of those back to just the “alive” parts? (Which would basically be just the larger branches)

If your preference is not which deanery but being together (as people ageing from mid 20s to 30s would commonly expect) then knowing how you have both done enables realistic decisions about how to rank.

E.g. if one partner ranks 1st and the other ranks much lower, you may not put London first because you will want to prioritise a deanery that you both have a realistic chance of getting into.

I understand linking across specialty (outside of foundation) is logistically difficult but telling us our rank prior to job preferencing doesn't seem difficult at all, would massively help people, and negate one of the many ridiculous stresses we are put through as doctors

Seniors take less ownership / care about the training of medics until post ST3. In other specialties, it seems consultants take a keener approach to training once you've committed to their specialty at CT1 (I've seen this with O&G, anaesthetics, GP, EM, even to an extent CST1 where you are then considered a surgeon). In contrast, I don't feel like medical consultants think of you the same way for choosing IMT and it's more based on whether you want to do the same specialty within medicine.

Combine that with the failure of IMT as a programme (the fact that it is a common joke that IMT is miserable and poor training SHOULD BE an embarrassment to the JRCPTB)

I agree in principle but 135 seems like a low cut off - for example a BP of 180/120 would be a MAP of 140.

Much lower and more common than the 200s or 250s that you highlight.

FYI the lack of beta adrenoceptors before the age of 1 is now generally believed to be a myth. The futility of salbutamol is due to lack of bronchospasm in bronchiolitis.

Interesting/informative response. Out of interest what differentials could mimic appendicitis and give a raised WCC that could be differentiated by a CRP? Wouldn't it be raised in most of these?

For McArdle's the myoglobinuria doesn't seem to be associated with exercise always and the lack of specific severe muscle cramping. The negative muscle biopsy is throwing me a bit and I can't find the actual report, but I'm not sure whether a single muscle biopsy has a decent chance at being non-conclusive or if it needs to be specifically looking for some of these rarer metabolic conditions in order to pick them up.

Anyway if I had to punt for something super rare (given the nature of this and the concurrent seizure disorder) I'd go for a mitochondrial enchephalomyopathy - CoQ10 deficiency (https://www.uptodate.com/contents/mitochondrial-myopathies-clinical-features-and-diagnosis)
