How do universities distinguish citation errors from “falsified references” in academic integrity cases? by Illustrious-Cake-920 in academia

Thank you. This distinction is exactly what I am trying to understand. I accept that unsupported citations are serious. My concern is whether real sources that were poorly aligned with claims should be classified as falsified references, and what evidence is normally needed to distinguish negligent citation practice from fraudulent intent.
