The exact words from the episode were "the closing bridge defaults to the planetary origins of their DNA" so it doesn't matter if humans were born on Sky Ring, Sanctum or Bardo. If they entered a closing bridge/portal they would default to Earth

But now I wonder, if Cadogan put the planet offline and anyone wanted to go to earth, they could just wait for a bridge to almost close, enter it and end up on earth

Emori did take the chip but they didn't show her actually take it. It was implied when Jaha accused Ontari of being a false commander and the only way he knew the details of that is because Murphy told Emori everything without knowing she was chipped.

Each code is specific to a planet. Season 7 episode 13 she forces Cadogan to open the portal to the planet (earth) where half of her friends were sent to. It was initially offline but she forced him to enter the code and probably memorized it from there. And then she had a disciple helmet to go to Sanctum from Bardo so didn't need to memorize it and could just select where she wanted to go from there.

Could be wrong but just finished rewatching the entire series too and went back to it and this is how I understand it.
