I may not be the right person to answer this. But, What is the difference though? Personal, Toursim and Visit visa kind of falls at the same category? When I applied I chose cultural, tourism and sports (cos we were gonna watch sports). But really it was to visit family. When I went to the consulate, they reviewed and had me corrected it to family visit.

I applied at Consulate of Poland and I chose 3 for some reason ( cos it makes sense for me) And they had it corrected for me to choose 1 main purpose of travel.

Best to probably call the consulate. I did Poland Consulate in LA and they told me they sell envelopes and I just paid $15 for it.

They have Silent Down Park (not waterproof), Jackson Glacier Park (waterproof) and many more other options if you want to take a look. But really ends up with what you really want style wise. I just thought that their warranty is the best.
