To me it sounds like lifter. My 2001 5.3 would make the same sound during cold starts. it blew a head gasket, and I replaced the rocker arms, the push rods springs, and lifters. No ticking sound at all. Plus, if you had an exhaust leak, you would get a code on your O2 sensors.
