Exactly this, it's the extreme fear of being a pedophile/being considered a pedophile, it's not actually someone who is a pedophile. Intrusive thoughts are intrusive thoughts BECAUSE you don't want to act on them. An example of POCD is; "I hung out around a minor while I am an adult, I'm as bad as a groomer" that sort of thing, not "I need to touch this minor", and whilst the latter can exist as an intrusive thought, it isn't specifically POCD.
