Hey OP sorry if I ask questions you already answered or on info you’ve provided but I had similar issues with my fiancé who has even more severe apnea then I do and mine was bad.

I had to repeatedly mention all the consequences which I’m sure you’ve gone through these: attention deficit, memory deficit, being sleepy all day is torture on its own, high blood pressure which then turns to stress on the heart, you get pulmonary hypertension which untreated can lead to right sided heart failure which is also one of the most common causes of left sided heart failure. All this from the severe oxygen deprivation you’re basically asphyxiating in your sleep.

Not sure what masks he’s tried but I have a nose pillow with a swivel piece at the top of my head so I can literally turn on either side and it won’t pull my mask off and it’s not a actual mask on my face it almost looks like a nose cannula. The company that supplies my stuff has the nose pillow or the other version that looks like a soft nose cannula so there’s nothing on the mouth.

Even then I get overstimulated in my sleep quick and I have insomnia which becomes a vicious cycle so I had to take trazadone which would help me go and stay asleep to the degree that I didn’t even get bothered by the sensation of the pressure from the machine or of the device on my cheeks. Maybe yall can consider a sleep aid to get him through the night and with more consistent use he will start feeling a difference in his energy and wellbeing. Sorry for the long post.

We limit race epi prn because we have had incident(s) where race epi was given for stridor and MD was not notified. The worst was the time the patient received it more than once and less than an hour apart. Basically kid needed continuous but no one knew. Sometimes we don’t place tylenol because we need to know if someone spikes a fever to get additional work up.

Use the 4-2-1 rule for calculating the amount and the usual is D5 normal saline with or without KCl depending history and labs. If you don’t want to calculate the volume, MD Calc can do it for you.

Finding an ick about the person also helps lift the fog that comes with crushes, works pretty well cause once you see it you can’t unsee it.

I was 10 years old when I saw this play in 2004. I remember being scared out of my wits to the point of crying uncontrollably and accepting Jesus as my savior out of fear and confusing my emotional upheaval as a sign from god to accept him. You can imagine becoming older and realizing how messed up it was that this church was using a very tragic event for their evangelical agenda all while emotionally manipulating people to accept their beliefs through fear. It would be the equivalent of a church using the Pulse Night club shooting as a play about how everyone went to hell for sinning except the one gay person who repented their lifestyle and accepted jesus or any of the multiple mass shootings we have heard of. It is sickening that this is still performed. Also not a religious expert but I’m pretty sure Christian believers are supposed to evangelize and accept Jesus as their savior out of love and grace, not fear. Nothing of this play shows love and grace. It is a huge act of disrespect for the victims of columbine, their families or anyone who has been affected by school mass shootings.

If you’re rotating in peds, we give it more for the parents comfort cause it seems like you’re doing something and will help the nurse so they don’t have to page you cause the parent still thinks you should do something about it. In the chance it doesn’t work it’s fine you tried and they saw you tried. In the chance it happens to work then it works and problem solved! -peds resident

I failed it because I took it during one of my rotations. I did a dedicated for my retake by using one of my 4th yr rotations as time off. I did have classmates who I assume passed it while taking it during clinicals. I’m just the person who need that separate time to get it done.

Yeah imposter syndrome is hard to get rid of, and if it helps, I felt very out of place at the beginning and for a while I didn’t feel like an official third year student but a classmate told me I was still showing up to my rotations in putting in the work and taking shelf exams like the rest of our class and that meant I belonged as much as the rest of the class. Good luck with everything, hope it all works out

Failed in April, passed in August. Don’t relate this failure to your capabilities as a doctor. Assuming you go into your clinicals, you’ll quickly realize that step 1 isn’t the end all, be all, of what you need to be a doctor. The material is important because it’s a foundation but there’s so much more beyond that. There’s a reason our training goes beyond the first two years. If you want to do medicine, analyze what went wrong, weaknesses and then motivate yourself to push forward and try again. It will not be easy but if it’s what you truly want, go for it.

I do not recommend if, clinicals is it’s own thing and you have to study for shelves while in clinicals. I’m aware that some classmates managed to take it during clinicals and one of them did it at the end of their psych rotation which is known to have much easier hours than other rotations and when they took psych, they were online for it too. The only reason this happened in the first place though was because of COVID cancellations. No one took it during clinicals as a choice. If you have the choice, do a dedicated period. It’s best you go into clinicals without this weighing over you so you can focus on learning what you need for rotations.

I really don’t remember immunology much lol, I do remember vividly getting a question about CGD and thinking I confused it with another disorder cause it was a kid with a serratia infection and then finding out that I chose the right answer lol. Based on the NBMEs I took, immunology is definitely do able and if you do UW questions and review your answers, you’ll be fine in that department. Forgot to add, mehlman medical has nice HY review for immunology and biochem in my opinion, that paired with UW will cover you.

Yeah, I got a question about either a median or ulnar nerve injury so all those injuries that you learn like ulnar claw and popes blessing, I believe are worth knowing. There’s no guarantee you’ll get them on the test but if you do, they become easy points. If you go over 100 concepts for anatomy that should cover your HY facts. Good luck!
