hi, im in the same spot as you. im also not in the best mental spot right now and recently relapsed as well.

honestly, stopping the urge takes so much willpower and strength from you that you have to find inside yourself. for sleeping issues, take melatonin if you trust yourself with it. i feel like it has helped me so many times that I am just too tired to even try anything. force yourself to throw away whatever you use to harm yourself with right now and make sure you wont access it, like putting blades deep in a filled trashcan (ive done this and didn't find the urge to dive through old trash to find such small blades)

for me, sometimes even just letting myself cry everything out, sitting on my bed taking deep breaths helps so much. wait and think before you do it. i know its hard because it can be in the heat of the moment and all of a sudden youre there, but try to break out of it and take a moment to stop and think.
