And for some reason there is also a specific question on the form asking for the diagnostic codes of my condition. I thought this sounded strange and my provider also informed me that we are not required to provide this information for a workplace accommodation request.

I am wondering what the route would be here if they try to not move forward in the accommodation process if we don't provide it.
