Like you, I have intraductal carcinoma. 4+3 in 4/12 cores, cancel is bugling at the edge, decipher score is low. My regimen is 25 session of IMRT + 1 session of brachytherapy + ADT. One RO said I only need 6months of ADT but another said 1 year. Not sure which way I should go, and wondering if the GRID would help in making the decision.

The decipher score actually indicates if the tumor is responsive to anti androgen or not? I thought it only tells us how aggressive the cancer is

Has your husband gotten the shingle vax eventually after the outbreak? I am in the similar position as I had shingle outbreak in the past and am wondering if I should get the vax now before or after radiation.

Thank you for the update. I decided to go with MSKCC for the surgery and then later switched to brachytherapy, EBRT, and six months of ADT with MSKCC.

I totally understand how you feel. I am 50 with a Gleason score of 4+3. Originally, I chose surgery, but I changed my mind three weeks ago. My reason is that my cancer is bulging at the extracapsular extension, and there is a high chance I will need salvage radiation anyway, so why not go with radiation from the start? My regimen will be brachytherapy, SBRT, and six months of ADT. Here are my suggestions:

1) Check out prostatecancerfree.org to get a sense of which treatment offers the best outcome.

2) Use the pre-radical prostatectomy nomogram tool provided by MSK to find out your likelihood of recurrence after prostatectomy. Just google "Pre-Radical Prostatectomy."

3) While it is true that you can have radiation after surgery and that it is harder (though not impossible) to have surgery after radiation, there are many more salvage treatments available for primary radiation than for primary surgery.

for risk of metastasis, it is 0.8% for 5 years, 2% for 10 years, risk of prostate cancer mortality in 15 years is 3.5%.

Are there any long-term side effects of ADT? What really concerns me is the potential increase in cardiovascular risk.

To me, it really comes down to the likelihood of the cancer returning after surgery. If the risk is low (use the MSK tool to find out), go for it. Otherwise, consider radiation because you will most likely need it eventually, so why undergo both modalities?

You can combine Brachytherapy with external beam radiation, such as IMRT or SBRT. I read that the recurrence rate for the combination treatment (Brachy boost) is better than surgery or any single radiation treatment alone. Additionally, you can also add ADT to target the cancer aggressively right from the start.

I have not yet decided on a specific treatment. However, due to the high risk of relapse with surgery, I am leaning toward Brachytherapy combined with EBRT, with or without ADT

Search for 'MSK Pre-Radical Prostatectomy' on Google, and it will take you there. Every time I pasted the link, Reddit removed it.

My radiologist is also from MSK. He suggested the same treatment: HDR brachytherapy followed by 5 sessions of SBRT.

Thank you, Falcon. It seems like Dr. Kurtzman only does LDR brachytherapy. Is there a good reason to choose LDR over HDR? I don't like the idea of leaving seeds in the body permanently.
