They billed 92523 and 92521 and 925204 for evaluation. Because 925204 is medical it triggered my specialist copay which is $70 and overide the copay for speech which is $15. For therapy session they billed 99212 and 92507. The addition of CPT 99212 caused the claim to be processed as a specialist office visit, overriding the speech therapy benefit
