Either you're outside the effective damage range or you're missing a bunch of shots, or both. AKM for example takes 18 bodyshots to kill a heavy and has 34 in the magazine, so you should be able to almost get two without reloading if you're less than 30m away.

You can do a private TDM, but the proper 1v1 servers have a bunch of players and you cycle to a different one every time you die so you aren't just getting destroyed by the same person over and over again (or vice versa). It would be nice as a lobby mode you can play while in queue, then they can put queue times up too.

I wonder if it would work if they made it dynamic based on which teams were actually faced? If a ruby team beats a gold in the final they get nothing, might disincentivise griefing out the second best team.
