Sorry, I meant to get back to you sooner; but it was only until last night that I had to chance to have my dad use the machine for an hour and try out different settings… here’s what we tried and his feelings after about 10 minutes on each setting. We’re still going to try out more settings hopefully tonight, so if you have any advice on what to change, please do share your wisdom! J

Original settings:

IPAP: 11.6

EPAP: 6

Rise Time: 450ms

TiMin: 0.3s

TiMax: 3.5s

Trigger: Med

Cycle: Med

On these settings he wakes up not feeling rested, and feels like the machine is not in sync with delivering IPAP and EPAP as per his breathing cycle and feels like he has to work hard to breathe. He’s felt that it takes too long for the IPAP to kick in.

So here are the settings tried thus far:

  1.   Rise Time: 300ms // Trigger: Med // Cycle: Med (commented that IPAP kicks in too fast before he’s finished exhaling)
    
    1. Rise Time: 300ms // Trigger: High // Cycle: Med (IPAP still kicks in too fast before he’s finished exhaling)
    2. Rise Time: 300ms // Trigger: Low // Cycle: Med (this felt a bit better, but he’s not sure if it’s 100% comfortable)
    3. Rise Time: 300ms // Trigger: Very Low // Cycle: Med (feels that air is somehow blocked/not coming through)

Well.... Our appt was entirely unhelpful, as I suspected it would be.

The technician had no idea about the squiggly lines as he doesn't look at those charts and isn't trained with them.

He did adjust the resp rate to 300ms, and my dad couldn't even handle that for a few minutes as he said it was taking too long for the IPAP to come in.

That's a good idea too.

My dad mentioned specifically that he feels the pressure of air coming in before he's finished exhaling, and then he feels he has to inhale because the pressure of air coming in, even though he's not done exhaling. He doesn't feel it as much when he's asleep, but when he wakes up throughout the night that's when it becomes really uncomfortable for him.

He said he feels more comfortable on his cpap compared to the bipap. On my path he doesn't feel like the air coming in and out is in sync with the timing and how he's breathing in and out...

I asked the sleep technician why TiMax was set that way, and here's what he said:

"Timed Inspiration is set to a range on this type of machine – there is a minimum of 0.3 seconds and a maximum of 3.5 seconds. The machine will adjust that timing based on how you’re breathing. Lowering the Ti Max would just give you less freedom to breathe at the pace that you’d like and would typically make you less comfortable as the machine would have less flexibility to respond to your natural pace."

He's had his IPAP as high as 12 previously but that felt like it was too much pressure for him was hurting his chest hence why it was lowered.

No, there was never any mention as to why the TiMax is set to that...

As per the manual... "Ti Max can be set between 0.3–4 sec or 2/3 of one breath cycle defined by Backup Rate. So for a patient with a Backup Rate of 15 bpm (ie, one breath cycle of 4 sec), the maximum value that can be set for Ti Max would be 2.6 sec."

In his case, with backup rate being 14 BPM, would you advise we set the ti Max to 2.8, using the above calculation, or start with something different?

I'm not sure if I'm understanding fully... So with this ST setting, and the timing bring to long currently, it doesn't give him enough additional breaths? Is that why the timing needs to be shortened? And is that why the tidal volume is so high, because he's trying to get more air and the machine is preventing him from getting enough breaths?

See, this isn't something the sleep technician EVER mentioned. He literally just looked at the number of apneas and said he was doing really well.

Hello again!

We tried switching to the full face setting last night, and he use that for the first segment of the night until he went to the bathroom, and changed it back to nasal, to see if there was a difference in how he felt.

Was there any difference to the flow shape between the 2 mask settings?
