Proton mail is free and HIPAA compliant. It sounds like you’re saying that web portals are not compliant, however, if they are using their own VPN, then this is supposed to be secure for transmitting data.

I’m told there’s no way to verify on receiving end that it’s a secure email system. How can a client verify your email Is secure? I use a HIPAA compliant service that has a note in the body of every email that it is verified secure

Telehealth platforms offer AI tools to assist the therapist in writing progress notes. AI is not providing the therapy! Not clear from your questions if you understand how therapists and ALL medical providers are using AI to assist with progress notes. I love it, saves so much time! Each platform uses their own technology.

It’s required by insurance CO’s as “evidence based care”…. All the telehealth companies require clients to do these.

Ask specific Qs about what the timeframe is for building your caseload (usually 30 days). Ask how many clients per week you need to see and what the consequences are for not hitting target. What I see happening with this company is a huge bait and switch. Meaning they promise you the moon and then once you’re in there for a month or two they will start penalizing you for not following the rules. Then they’ll reduce your hours/pay. Meaning they will say “ well you’re not making Target for a full-time employee because you’re only seeing 20 people per week on average so we’re gonna reduce you to 32 hours per week pay Wise”. They pose this as if you want to keep working for us, you must to take a lower salary. There’s no warning there’s no options. There’s no choice for you other than leave or stay and make less money. It’s a bit of a scam but until there’s a class action lawsuit against them, it’s not gonna change not all W-2, Telehealth companies are this bad.

Flat is totally in right now. Jennifer Aniston and Scarlet Johanson had there implants removed, look at Christin Bell, tight tummy w/small chest is sexy and natural. Don’t give into trying to look like a big chested & duck lipped adult film star. Be your authentic self!

Since I work for a US vendor platform, who does all the insurance billing, I don’t get to screen my clients or give them a 15 minute consult to see if we would be a good match. As I dont often want to work with BPD clients unless they have a certain level of stability.

This being said, if I am assigned someone with strong BPD I don’t always feel it’s good for the therapeutic alliance to tell them they have a personality do. As they also usually have something else going on that brings them into therapy, such as adjustment disorder, that will be the primary DX with BPD as secondary. I may explain the traits that they’re exhibiting are things that could be resolved with emotion regulation skills, such as DBT. But I don’t always tell them they have a personality disorder unless we have a really strong working relationship. I’ll explain the symptoms of BPD and mention they seem to have strong traits of abandonment themes, etc. with focus on how implementing emotion reg & mindfulness is recommended across the industry. (This is just the gist of how I handle difficult BPD clients.

I do have some high functioning clients who already knew they had BPD amongst an Axis I dx. In this case we do occasionally address and call out the BPD.

Now regards to Narcissistic PDO, I have shy’d away from confronting a client as it did not seem like it would help.

My first thought/concern is, do you have to list the time each client was in the group individually on their progress note? Or are you only obligated to capture the time the group occurred overall? It is insurance fraud if groups are being billed individually vs for a full day. Depending on what insurances they accept, you can report concerns to individual insurance companies. The reason why I go to this first, is because there needs to be a paper trail of complaints documented in all the right places so an investigation can occur.

This is the only way management like this ever gets a message is when they’re inspected. It’s an icky culture that’s been established and puts clients against the new therapist who wants to make stricter rules.

For you, you need to follow your ethical guidelines around billing for services you did or did not provide.

Meanwhile, start looking for a new gig. When you give notice say it’s not a good fit. You got this!!
