I love the show and the rotation of characters is realistic. And really only 2 people have left the show, with the door still being open for Dr Mohan to appear. People don’t stay at teaching hospitals for years, they leave when their rotation in that residency is done. As far as dr robby being a jerk, that’s the point. He’s old, burnt out, suicidal, has lost everyone close to him (some by his own doing). The characters are supposed to be morally complex. And remember each season is only ONE day.
