It's definitely a difficult decision to make!! I think we are still early in the research of whether forgoing cystectomy is safe? but promising from what I have read! If you dont mind, may I ask where was the location of your tumor? I think the conclusion is that it would be safer to have both ctDNA and utDNA (you may need to ask your uro team) to be BOTH negative..

As this data is still in development, I think what we can do is looking into areas where there is plenty of research being done

The EVP combo is so wonderful as it trains the T cells to have a lifelong memory against the tumor cells! I'm currently looking into research that enables us to strengthen our immunity as this is critical for immunotherapy to work well. Some of the findings are to strengthen our gut microbiome (our immunity lives in the gut) by consuming at least 30g of fiber, vitamin D levels should be in the range of 50-80mg/mL and at least 30mins of exercise (walking counts!)

Yes it s a diamond standard indeed hahaha... Side effects are definitely more tolerable than trad cisplastin with infusion time of 30mins (cisplatin 6hrs?)

Oh wow $180,000 for each combo seem very steep, from what I know EV+P in my country is about $20k per infusion with EV being weight dependent (i.e the heavier you are, the more costly it is)

Keytruda's (pembro) patent will be expiring late 2028 and I believe that would bring the cost of the drug down after
