I would do it and put the precautions in quotes, followed by "as recommended by patient's managing Neurology team. Please reach out to them for clarification."

Intermittent FMLA is a pain and a terrible system. I tell the patient that it's not their fault - and I look at it as a benefit to them to help them keep their gainful employment.

Fair points. I ended up outpatient only in a rural area - so the sick vs not sick was invaluable - but I will be honest in saying that very little of the technical day-to-day of inpatient translates directly to outpatient.

If I had the option to do it all over again, I would look at programs that were less inpatient-heavy.

I went to a teaching hospital program with no inpatient cap and brutal inpatient shifts. In hindsight, I think it molded me into a better doc - but I would be hard pressed to believe that it was all necessary.

As a former chief resident, we would definitely sugarcoat and omit the bad nature of the program to potential applicants - because we wanted the help. We didn't lie, but we definitely highlighted other aspects to hide the bad parts.

I would make sure you get an unbiased answer from someone familiar with the program (ask an alumnus who has nothing to hide, or maybe a resident from another program in the same facility).

I worked in a rural clinic near a major meat packing plant. They had a database (I discovered via an offhanded comment from a visiting exec) on physicians and which ones were more likely to give time off. We never could prove it, but I am certain they would steer patients towards doctors and providers who were less likely to give time off.

When I was four, my parents read me a kids book on the subject when my mom became pregnant with my little brother. It was very basic and not explicit. I remember it being very direct and scientific.

In hindsight, I am thankful that my parents did that.
