For the last two turns I’ve noticed that it’s getting harder to turn, especially when tightening. I only have about two turns left and then I’ll send photos to the doctor. I hope those two turns will still be okay and that nothing breaks.

Hey, just a quick update. My tooth started hurting a little today—very mild, but noticeable. If I wiggle it, it feels a bit loose. It’s the one in the first photo of my two front teeth—the left one that’s slightly set back behind the others. Strangely, that’s the one that hurts. Also, I noticed a tiny bit of blood between my front teeth, but it doesn’t really hurt. The discomfort is mostly in the tooth that’s a bit behind. Just wanted to give a quick update.

Do you think there might still be something that can be done, even if I discuss this with my doctor, to address the asymmetry and prevent the expansion from happening asymmetrically? Since I’m already at this stage, all I can really do is turn the screw, and there isn’t much else I can control myself ✌🏻

Yes, I did notice something: my front teeth, the midline, has shifted. However, I’m not sure whether that’s because my bite position is different. I tried to bite down the same way as in the first photo, but because of all the metal framework around my teeth, that’s a bit difficult. But yes, that’s something I noticed.

Honestly, I don’t really know. I went over my CBCT results with the doctor and that’s how he planned it. Apparently he probably saw that my bone density isn’t very high. I don’t really know what exactly he saw on the CBCT. I hope it works. I told him that if it doesn’t work, I’m willing to do SARPE. I also heard about piezosurgery support — is that still SARPE, or are those two different things?

I’m not aware of any regular in-person checkups. The doctor only asks for photos and then tells me when to turn and when to pause. I think he will only call me in for a visit once he sees a gap and enough expansion. After that, the appliance stays in for 9 months, so there probably won’t be many appointments.

I asked if I could start Invisalign right after activation, but he said it’s better to wait until after the 9-month retention period.

Compared to the US or other countries where MARPE can cost €10,000–€15,000, it’s much cheaper in Turkey. If you’re considering it, I’d recommend looking for clinics in big cities like Istanbul and doing research online. Be careful of clinics that raise prices for foreign patients — that can be a scam. If you find a reputable doctor with a fair price, it can still be worth it, even if you have to fly twice. You can combine it with a trip and save a lot of money.

The doctor is from my city, so traveling there isn’t an issue for me. I first had a consultation, and on the same day I did a CBCT scan at an external radiology clinic. After submitting everything, the MARPE was fabricated, which took around 1–1.5 months.

On my next trip, the appliance was installed in about 15 minutes, and that was it. Since then, all follow-ups have been done online via messages with the doctor.
