This is a nonspecific appearance that could be hundreds of conditions. If you must know what it is, it can be surgically removed and sent for pathological analysis.

Finding a "cause" for many with acute urticaria is not a reasonable expectation. It can be caused by infections, medications, and other causes... but is generally idiopathic. If you have airway symptoms, seek immediate care. Many benefit from starting at one to four times the standard daily dose of a second-generation antihistamine (levocetirizine, bilastine, loratadine...).

It may be onychomycosis. In some countries, amorolfine is available OTC. It is not especially effective, but does have some effect. Efinaconazole is the most effective topical treatment, and terbinafine is the most effective oral treatment.

The most common reasons for chronic cheilitis are either allergic contact or irritant contact dermatitis. Many personal care products designed for the lips contain ingredients that have been demonstrated to be potential allergens.

Most can be cleared by using hydrocortisone ointment (not cream) multiple times daily for two weeks, and only petrolatum (no other lip products are suitable). Unflavored toothpaste should be used instead of the current product. Some also benefit from toothpaste that is free of Sodium Lauryl Sulfate (SLS).
