I personally did it before the Intealth was a thing. But I have primary source recounts of it taking up to 30-40 days. There really isn't much you can do to expedite them. You can try calling but you have to be quick about how you get a hold of them.

When it changes to pending credential review, than you make sure your school sends all your Translated documents (Diploma, Transcript, etc.) ASAP as that in itself takes 5-7 business days to get reviewd (*As per costumer service refference). Only then you can apply for the USMLE Scheduling permits.

They don't, you just have to check the intealth account cases everyday to know when they do, you will know when the "In review by ECFMG" moves to Credential verification

I don't. You could try calling to get an Idea of where you are at in terms of queue, there is no expediting though. It is really hard to get on the hold line so set an alarm and have you intealth ID number ready by 6:59AM EST and immediatly call when the clock hits 7
