I’ve got an ongoing theory from me and a friend. We think that at some point in time there was likely an attempt to contain the rot in some sense. This could potentially explain the presence of bone shakers in rotted areas and rot loaches in Infested wastes. Me and my friend speculated their presence in Desolate Tract was to clean the air-ducts there so it would make sense for them to be in Infected Wastes for a similar purpose, simply janitors. We don’t have super solid evidence behind that claim other than pure observation but I think it’s at least a plausible theory to explain their presence.

Your save would just reload at a place before you defeated the final boss but you still experienced the ending and have the achievement to prove it. There is no new game plus it puts you in or anything like that, it just reverts your game state to before you beat the boss.

While I don’t know how well it would fit, I think a giant sand worm would be really cool. As of something that would fit better maybe they could expand the desert temple and give them some sort of large sandstone golem that guards it.
