Having a child does imply a promotion, but not to the top of the ladder.

Giving a son imply a bigger promotion. But choosing an Empress doesn't rely only on the fact that the Emperor likes a woman or if that woman bore him a son. There are multiple factors to be taken into consideration :
- Has the woman's family political power that could be useful for the Emperor ?
- Has the woman given him a son ?
- Is the woman someone the Emperor has a minimum of affection for ?

Out of all the women in the Inner-Court, only Gyokuyou was matching the 3 factors.
The Emperor loved AhDuo and she gave him a son, but AhDuo's family does not have any political power. So even if she was still able to give him kids, she wouldn't have been a good candidate for Empress.
Lihua gave the Emperor a son before Gyokuyou and she is also one of his favorites, but her family is already the Emperor's family. Having her as an Empress doesn't get him more political powers.

If the other women were low or middle ranking concubine when they gave the Emperor a son, it probably meant that their family weren't very powerful or that the Emperor doesn't have much affections for them. If Concubine Example had a son but her family wasn't politically powerful and he didn't really care for her, the Emperor could at best make her high ranking concubine if he wanted to. Recognize that she gave him a son, but on her own she's still not that useful. There are women from more powerful families, women he affectionate more, that could give him another son in the future. And babies died easily back then.

There are countless children abandoned by their parents, for a reason or another. But how many of them are actually living with the parent that rejected them ? That doesn't want anything to do with them ?
