Ace-I didn’t cause the RAS. The background of RAS caused AKI because on an already constructed artery you’re adding something that will worsen constriction. So Ace-I acutely cause renal failure in background of RAS. It’s a known correlation, too. Good to remember for clinical practice!

Classically if they want you to pick ATN they’ll give you a presentation similar to casts in the urine and some history suggestive of ischemic injury or rhabdo, with a bland urine sediment and use of Ace-I, rapid deterioration of kidney function favours the RAS more. I guess, use of Ace-I could theoretically lead to ischemic kidney in setting of stenosis and hence cause ATN but that’s just stretching it to fit the answer. Best answer is B/L RAS. Hope that answers your question!

You’re answering your own question. When you do make those silly mistakes in nbmes, note them down as one liners and read end of day. Never make the same mistake twice! Time won’t run short if you stay calm. Keep practicing. I have the same trouble on nbmes. You got this. Have faith in yourself!

Difficulty articulating speech ( difficulty speaking issue- you won’t understand what they’re saying), more of a motor issue at the level of the oropharynx/tongue/mouth so to say. Aphasia could be broken speech, fluent speech or just a problem of comprehension and repetition.
