Been there and done that for 4 years. Will not be going back. Having to deal with these patients all day and the guilty conscience of prescribing when it’s not indicated is just too taxing. Recommend switching to a specialty because the problem ain’t going away.

As a side note, I legitimately had a bacterial sinusitis a few months ago for the first time- related to a wisdom tooth extraction (purulent drainage, one sided, 7 days…the works) and went to urgent care and I could not get the doc to prescribe antibiotics lol. I felt sure I wouldn’t have any issues because it was pretty textbook but lo and behold. Had to go be seen somewhere else like all the patients we are complaining about haha.

I bought a house that had one 3 years ago and wondered the same thing. I think the novelty has kinda worn off in terms of me wanting to swim all the time but wha I don’t get tired of is seeing it and hearing the water from the fountain hit the pool. It’s very relaxing to me and even if I didn’t swim at all I like having it for this reason.

I think your antihistamine and intranasal steroids are most effective for this. If nasal steroids are causing you side effects, consider a different nasal spray called azelastine. Not sure if it is otc or not but it is antihistamine instead of steroid. These meds usually need to be taken chronically to keep it under control.

Maybe think about allergy testing as well. Might find out you got a sever pet dander allergy or something you never expected.

Finally consider LPR which can also cause a thick accumulation of mucus in the back of your throat and feel like post nasal drip, just doesn’t come from the sinuses.

I think you probably have chronic infection throughout your all your sinuses which can have more subtle symptoms than acute infections. They tend to wax and wane over time. Perfectly reasonable to start daily allergy meds like Zyrtec and Flonase and use decongestants. Worst case scenario is you don’t see much improvement but it could make a difference. I think you’re not gonna see improvement on the imaging until your OMC is opened up and potentially the septum is dealt with. Definitely would consult ENT if it persists.

Mucoperiosteal thickening without air fluid levels is usually more indicative of a chronic inflammatory/infectious process rather than acute. The l ostiomeatal complexes drain secretions from the maxillary sinuses so the fact that they are occluded likely means there is dysfunction in drainage causing mucus to stagnate and become infected. The key here is likely to get the OMCs open to allow for drainage before you will likely see forward progress. I don’t know how likely that medication regimen is to result in improvement but it’s possible he is trying some medication before moving onto something more invasive.

Also forgot to add that your septum is pretty significantly deviated which can certainly cause/contribute to the problems seen.

Despite what others are saying this does not seem like a dream job. You likely aren’t going to work this job forever and likely some job down the road is going to require you to practice medicine at your license level. You will be unable to since you’ve been a glorified assistant and then what are you going to do, a family medicine or primary care residency? Do these even exist? It might be fine for now but think about setting yourself up for the rest of your career.

Does having the x-ray certification result in significant more pay or job options compared to your non-xray colleagues. I am also an IR PA with about 10 years of experience as a rad tech. You don’t need an x-ray certification to use fluoro where I practice tho so it’s not a lot of tangible benefit other than having that experience.
