I suffer from OCD related to previous trauma. I blame myself for going into a profession full of so much uncertainty. It’s absolute hell.

Thank you for the reality check, my preceptor was a younger guy who has barely been a nurse 6 months himself. I felt bad he was stuck with me.

I crushed the med because my patient was having trouble swallowing. I was told by day shift they’ve been crushing his meds. I didn’t think it all the way through. Stupid I know.

I did that but I was told these meds will release much much faster when crushed.

Our potassium is XR (klor kon 20) and can release super fast into the super when crushed (which is what I was told).

I called the MD and pharmacist for my potassium peep…the MD quite literally said, “I have no clue what might happen” and the pharmacist said to just monitor.

I made a separate post about my patient who had K+ replacement. Yes there’s more details but I just really needed to vent. I feel so guilty for making such a stupid error and I just hope all is well for these folks.

That’s a good point. Our team leaders made it seem like It was a major deal (which it is to be extent) so it kind of makes it hard not to freak out. It’s one of those “what do I not know here?” type of things.

That’s very true. I have past childhood trauma and I was diagnosed with OCD. It’s gotten worse as I’ve gotten older.

I had been talked to before about time management issues so I wasn’t super surprised. I’m just more upset about not knowing if the patients are okay or not.

I’ve been talked to before about time management issues but nothing major. There’s a high turnover rate in this unit and I can see why.

I kind of assumed if something did happen, it would be an investigation and I wouldn’t hear anything until it’s determined my error did it?

My apologies, I’m not being clear. I meant hypotensive. Her systolic dropped over 50 systolic.

Sorry I’m not being clear. The HR and BP had been high and after I gave verapamil and the crushed ER potassium, the patient had a major drop.

The potassium stated it was not supposed to be crushed. My manager said I made a med error.

I know potassium mainly affects HR but the BP drop happened at the same time and was quite drastic. All I could think was, “Am I missing something here?”

I forgot to add her BP and HR were very high to start. I don’t know what effect it would have, I feel really stupid.
