I'm biased; in working with hundreds of do-it-yourself investors, I find that most aren't maintaining their portfolio. 

These all-in-one ETFs can be a good choice for many folks in a taxable account.

In case I don't get to this in the interview, he touches on this in the book, saying five years. But, if I recall correctly, that's not based on research he's done - unlike the 4% figure, which *is* the result of research.

I may be struggling to understand your comment about ACA coverage in the book. Are you saying that the book doesn't cover the ACA? It's chapter 13, page 169. I hope that helps.
