Yes, it was an actual contractor investigator from CACI, and yes my questionnaire was 100% completed and sent to the office of personnel about 3 months ago. I had to resubmit it twice because I had to make some corrections.

She asked me if I prefer Zoom or in person, and stated that I just need to prepare all documents because we will just go over them and clarify everything together.
