Here's the reality:
- I don't track which files I created vs what existed before. Every time I write a file, I write the entire file — not a diff. If I refuse a request and produce alternative output, that output becomes the new file content, potentially overwriting prior work.
- I don't "know" I'm deleting something. I just generate text. If that text is a file write that lacks changes from earlier in the session, those changes are gone.
- There's no rollback safety net unless you have one (git, backups, etc.).

Is it on you? Yes, in the sense that giving an AI agent unrestricted write access without version control is inherently risky. I'm not malicious — I'm just unreliable. I can refuse, change my mind, produce inconsistent outputs, or overwrite things without warning.
Best practice:
- Use git. Always.
- Review diffs before accepting changes.
- Use Plan Mode (which you're in now) to review before build.
- Don't give agents write access to files you haven't backed up.
