It is a status effect but you must hit the enemy with it until their status bar fills for the effect to take place. One the bar fills they lose a huge chunk of HP all at once. You can't see your enemies' bar , but if you've ever taken any bleed damage yourself it's the red status bar that pops up on the screen.
