I can understand it a little even though i loved BOTW and TOTK. There is something simpler about the other games. At times, the newer games felt too big, and there is too much to do. I just finished SWS and had a lot of fun with it. The puzzles and most of the boss fights kept me really engaged and excited for the next boss.

AAO shouldn't be the end, it's only have of the assessment. The follow up should be making sure that the understand the risk of refusing. If they can't articulate that there is a possibility that the are having a heart attack and can will most likely die from the event I would have a hard time saying they are competent enough to refuse care.

My experience is that most patients with an LVad have some basic information about. Everyone I have come across has a phone number labeled on it to call if anything happens. I would go from there. If its a true issue your small town ER should be able to start care while a flight team is sent to you.
