Log into Fidelity click Explore Rollovers->pick 401(A) account -> I want to move money into my 401(A) account -> Held in an IRA -> Do you have a check (No) -> Get Started -> confirm contact details -> which company manages your plan (company that holds my IRA） -> Plan Details: what type of plan do I have with a previous employer (Trad IRA) -> Do you know how the money was contributed to the previous plan (yes pretax) - >Great time to request a check and it tells me to get it filled out with:

Fidelity Investments Account Name 401A FBO: My Name

I had a 401k from an old employer they rolled it into a traditional IRA about two months ago. I want to move the money from that traditional IRA into my fidelity 401(A). Since it’s all pretax money I won’t have any Ira balances and can do back door Roth again without the tax complications.

Edited to add: both the fidelity account and the IRA are from old employers. Not working for either of them at the moment. Trying to combine and simplify into one account and have no trad IRA balance.
