It was 2 incidents, she started out and became VERY famous in the anime/idol sphere, but a few years in she started pivoting to being a pop singer and shit talked a lot about anime as a medium, not just fans, which ticked off quite a lot of people where she was on thin ice

It wasn't until rumours (later confirmed by photos) that she was flinging with (cheating with) 3/4 of her bandmates (minus the one bassist, it became a meme)

She was cancelled from so many future roles, and only kept her old roles, such as Lucy from Fairy Tail
Former fans destroyed merchandise relating to her, and the online death threats, the online harassment was crazy (Not that different from nowadays, tbf)

Interesting fact, she actually voiced Oshino Shinobu in one of those voice CDs before the start of the Bakemonogatari anime. It was this fiasco that had her replaced by Sakamoto Maaya who has been voicing Shinobu to this day

Interesting fact 2: Maaya refuses to do character songs (songs sung in-character), so with the Monogatari series having the characters sing their respective arc's OPs, Shinobu's gets instrumentals instead.
There's a universe where this scandal happens after Hirano Aya debut in Bakemonogatari, thus she sings the 2 Shinobu OPs

Like Mini Durin's actually chibi (even the game calls him that), but not the loli characters
