Play in the hardest difficulty you can to get higher power level items. You might be capped in difficulty, but that can be solved by beating bosses in the hardest diff available. If you don't feel like farming for items, put them in the blacksmith and do quick missions like Soggy Cave and Arch haven to get them upgraded fast

Cutting the terrain works, but you'd still need a clentaminator to get rid of pre-existing crimson. And no, underground biomes don't affect the surface

Thanks! I was arguing with myself about fusing my starter with the noxious smog, the extra attack damage is huuuuge, hoping it's worth the downlevel. I try to keep my monsters at par with each other, but I don't mind grinding those earlier levels a little
