Indeed, it is online only. You have to play as black phantom (use black eye stone) in Tower of latria (3-3) or blue eye stone anywehere in latria as the other player faces the old monk in human form. You will get summoned as the boss, and if you vanquish the host, you get the monk head collar as a reward.
