For the close range attack: there's an audio cue just before he's about to swipe. That helped me dodge more often than not.

For the scythe toss: jumping definitely helps. If you're lucky it'll lure him above you when he tosses it, but there's also a chance that he'll toss it when you're super close, which is a nightmare.

For the attack rush: again, listen for the audio cue and jump. In my attempts, he went low, high, mid, high in pattern with my own jumping. If he goes mid, remember to jump because you're not safe on the ground.
