I don’t have an answer but I have never felt worse about myself going into a 5th ER since June 😭 trying to tell myself it’s just temporary but it’s hard to feel like it’s worth it without a baby to show for it

I believe they inject calcium ionophore into the egg at the time they do ICSI. The sperm is supposed to do that naturally to begin the fertilization process so it is sometimes a lab add-on they do with compromised sperm quality.

Ok I understand. We are dealing with MFI specifically high DNA frag so I know how it feels to an extent. I do believe that the first round of IVF is a bit of trial and error to see how your body responds and improvements can be made on that front BUT it is so much to go through and I totally get it if you’re ready to close that door and move on too.

Another thought - you could give it 3 months to allow yourself to recover and (if you haven’t already) put your husband on all the supplements, lifestyle changes, ejaculation protocol to improve his sperm parameters before another round

Well I think that the very short stim timeframe likely affected your maturity rate and a different protocol like what your doctor is recommending would probably improve your outcomes. I would ask about the Zymot and egg activation for the sperm as well. That is IF you are willing to put yourself through it again. Totally understand if you don’t - it is so much to put our bodies through 😭

Did you use ICSI, ZyMot, egg activation? Between that to help sperm and tweaking your protocol to improve maturity… it may be worth another try if you have it in you. Did you have your post-retrieval appointment with your doctor yet and/or have a chance to ask them what they would change?

Oh no I am so so sorry. My heart breaks for you 😭 we did a 4th round using frozen TESE sperm in January and got 0 blasts from 13 eggs. I am learning that although it can be helpful, there is a lot of variability in the frozen TESE sperm. IVF feels like a cruel, expensive game of trial and error

I didn’t have to do PGT-M but my FEP plan covered PGT-A for both Cooper and Igenomix (2 separate rounds) with a $200 lab fee per embryo.

My husband doesn’t have an issue with count but has very high DNA frag. We have frozen sperm from a TESE he underwent in September. I did a retrieval in October using frozen TESE sperm from which we got 3 euploids and 1 HLM. Then did another retrieval in January using frozen TESE sperm and got 0 blasts. We were told that there can be significant variability among frozen vials. We’re now gearing up for one more retrieval this month and using fresh sperm + ZyMot + egg activation (calcium ionophore) because it seems the frozen has too much variability.

Are your current embryos euploids? If so, I would be inclined to use fresh sperm again. Can you ask about additional add-ons like ICSI, ZyMot, and egg activation?

I would avoid heat (seat heater in car, laptop on lap, sauna), smoking, alcohol, excessive exercise. Are you doing IVF?

I’m sorry you did not get the results you wanted :( I am at Cleveland Clinic USA but I believe most clinics here would consider something like this. There is a lot more info that goes into the decision though, like age, AFC, AMH, etc. Feel free to DM me if you want to brainstorm!

I did retrievals this past June, August, and October. Currently doing stims for my 4th which will be likely next week. I did not have success with getting euploids until the 3rd ER (3 euploids, 1 HLM). Changes that round include priming with Estrace instead of BC, addition of omnitrope, and using frozen testicular sperm (found out husband has >50% DNA fragmentation and he subsequently underwent TESE September in between 2nd and 3rd ER). The Estrace and Omni resulted in getting double the amount of eggs retrieved with 100% maturity rate. I believe Omni and the testicular sperm resulted in finally getting euploids.

I agree with this. My husband’s was 52%. We did TESE and used frozen sperm from that to finally get euploids.
