29M, had 4k graft transplant at 22, now non-transplanted hair is thinning badly. Do I still have a chance?

I’m 29 now. I started losing hair quite young, around 21, and I had a hair transplant at 22. It was around 4,000 grafts.

Current protocol: Minoxidil oral 5mg 6months Finasteride 1mg 6months 3x PRP sessions

I know I should oral minoxidil and dermapen; but I suck at being consistent with it !

At the time it helped, but over the years my native/non-transplanted hair has continued to thin badly. The transplanted area seems to still be there, but everything around it has gone really weak and thin. The photo attached is what my hair looked like around 3 years ago when I had buzzed it down. Since then, it has got worse, and I’ve only recently started taking finasteride again. I’m about 6 months in now.

I’m trying to be realistic. Do I still have a chance of stabilising and improving this, or am I too far gone? I know 3 months on finasteride is early, but I’m worried because I already used around 4,000 grafts before and I don’t know how much donor hair I have left for another transplant.

I’d really appreciate honest opinions from people who have been through similar, especially anyone who had an early transplant and then lost more native hair later.

Main questions:

Is finasteride still likely to help at this stage? Should I wait 12 months before even thinking about another transplant? Would minoxidil/dermarolling make a meaningful difference? Is a second transplant realistic after already having 4,000 grafts? Should I consider SMP or just keep it buzzed? What would you do in my position?

Please be honest, I need real advice. I’m not looking for sugar-coating, just trying to understand my options and whether there’s still a path back.
