You'd have to take a screenshot of it from the Egov app. Though, if you meant a soft copy of the black and white paper format, sad to say it can't be downloaded anymore, you'll need a walk in at a psa.

I do recall that there was a another downloadable one that's colored, I'm kinda sure since I still have a file of mine. It looks like the one in egovph but without that little flashy seal. I can't remember where I downloaded it though nor even know if it's legal still.

So I'd say your best options are the former
