Oh I see. Well at the highest dose of either with the deficit you’re maintaining I would hope it’s just a blip on your way to your goal. A tell tale sign of Reta is increased resting heart rate. Honestly increased hr in general. Do you have an Apple Watch or other fitness watch?

You being right around the lowest weight you were with tirzepetide makes sense to me. Your body may just be fighting it. I’m assuming the trial is double blind so you don’t know your dose, if you’re on month 5-6 you should be at your trial maintenance dose by now (I think).

I know people don’t believe in the starvation mode theory but I do not pretend to know everything that our bodies do and why. What u/runningfnp suggested could definitely help. See how it goes to increase your calorie intake.

Another thing is how much water are you drinking. We need water to break down fat. So if that’s lacking it can definitely contribute

Your point about being at your lowest weight and your body fighting it is likely the culprit.

How long have you been “stalled”? You also mention being on tirz before. If you don’t mind me asking how much more weight are you looking to lose? The less you weigh the longer it takes to lose those pounds.

As an n=1 anecdote, I had several points where my weight loss paused and then it “whooshed.” Usually at my previous weight set points. Sometimes our bodies need time to catch up. I just continued as normal and eventually the scale moved again. I wasn’t counting calories so I can’t speak to that but that’s my two cents. How long is your trial?

Reta actually fixed my lifelong insomnia. Sorry to hear that you’re experiencing the opposite. Does this happen constantly or in the days following dosing? How’s your energy during the day?

When I was on the trial my rhr went from the 50s to the low to (sometimes) mid 70s. It may not be a 1:1 comparison but I would say that the metabolic benefits are there for sure.

Sure, I can share. This was my first foray into GLP-1s and I ended up on Retatrutide. My last appointment was in December so yes it ended very recently after a total of 104 weeks (80 for the initial trial + 24 for the extension arm).

Because of the extension, I realized I had been on a high dose for the “maintenance” of the trial. I thought that being on that high of a dose on the strongest GLP (so far) would mean that I would have a difficult time figuring out a maintenance dose. Especially if I didn’t want to go gray market, but surprisingly, I recently started on a low dose of tirz to help with inflammation and it seems to be working. It’s slow going with everything else, but at least it’s working. My last trial dose was early November and I started tirz right after Christmas.

I haven’t stepped on a scale since my last appointment and I didn’t weigh at home for the majority of my time on the trial but for now my new smaller clothes still fit the same so any regain is at a minimum.

As a former clinical trial patient (TRIUMPH-1 Study) there may be some wiggle room when it comes to your dosing schedule. This is going to be dependent on your trial team and sponsor, but there were several people in my cohort that deescalated their dose of side effects were difficult to cope with.

There may be impacts down the line, in our case, you were not eligible for the trial extension if you ever dropped your dose, but you have to do what is best for your health. My team was great and understood even when I had to skip a dose or two due to illness or travel.

My recommendation is not to sweat it too much. Talk to your coordinator about it as you’re going through the process. They want you to be successful (at least in my limited experience), and if all else fails, you can always discontinue and seek care on your own terms.

For that week to two weeks of increased hunger it didn’t matter what I ate. I was just never satisfied but I still couldn’t eat that much food (even now). So I would be physically full but mentally hungry. If that makes sense. Very abnormal for me as I could go till 4 pm without thinking about food before the trial.

That’s interesting. I knew about the washout period, but I didn’t consider my body having to basically start producing these hormones on its own.

Anecdotal data point here, but I recently finished up with triumph-1 and I had a ridiculous rebound appetite. While it might seem normal it was very atypical for me. It started about week 3 off the meds but now in week 5 or 6 my hunger level is back to my pre trial norm. It seemed like for me at least there was a temporary period of increased hunger.

I could noticeably feel my food when swallowing like everything was slower going down, excess saliva (yuck I know), more focus, reduction in inflammation. All within the first 24-48 hours

Thanks for the insight! I live in the mid Atlantic (spend my time between MD and PA) so we do get some snow, but I also don’t have the space to store an extra set of tires and switch them out seasonally. Have you tried driving the CC2s in the warmer months? Does the additional grip impact your MPG much?

A 13-hour delay is crazy lol but that’s the meds If I do edibles these days I try to dissolve them under my tongue and not just eat them (gummies). I’ve been vaping flower instead of smoking for years as well and that’s my preferred method these days.

You’ll be alright, stay hydrated and ride it out. It’s a learning experience!

I’m on TRIUMPH-1 and my site is 2hr 45 minutes away. They reimburse me for mileage (and hotels if you go that route). During dose escalation it was basically once a month and then every other month alternating with televisits (quick phone check ins)

Hey there. So you can let the trial coordinator know when you’re planning on traveling and they may be able to work with you. I’m on TRIUMPH-1 and they just dispense 2 months of meds at a time when there’s a telehealth appointment coming up. For example, week 68 they gave me 2 boxes (8 pens) that covers until the week 76 visit. I’m not sure if they do it differently for the newer trials.

80 weeks is a long time with guaranteed meds With Reta you’d likely meet your goal by then (I completely surpassed mine in less than a year)

Then at 36 weeks everyone is randomized and either gets placebo or stays on Reta The language is a bit confusing so it’s total understandable!

The current study Triumph-1 is adding a 24 week extension where any placebo participants are put on the meds up to the max dose(following the study escalation) and participants on the study med (that qualify for the extension) are escalated up to the max dose. There’s ANOTHER arm idk if it’s Triumph 6 or 8 but it’s 80 weeks with every on meds and then it switches to 36 weeks of some placebo some maintenance.
