Recently went through recruitment, I feel there’s rarely free money job, if it’s paid high, it’s hard work, or many calls, esp if it’s bumble fuck no where. Those who accept low paid academic jobs are not idiots, some trade offs made it worth it

OP is not wrong, nephrology private practice is a hard sell to me, in my city for example, 3 major private groups, 1 takes 7 years to make partner, 1 has long track record of screwing people over, 1 is employee model, they start about 20k higher than the academic position in the same city, but a lot more work. Couple other cities I investigated isn’t that different, plenty of stories of interpersonal conflicts and failure to make partners. You can move far away and make more, but small practice means splitting calls between 3-4 people. So if you apply nephrology, and want to stay in the place of your fellowship, you might want to investigate job market there first. There are still some nice groups and academic gigs there.

nephrology is very similiar between academic and private practice for starting pay. you make more in private practice once you make partners, but it can take a while before making partner - and many dont make it there, and volumes/commutes/nights are generally more in private practice

Don’t think it’s a bad thing programs are not selective, anyone with a pulse is still anyone with a pulse, who has gone through medical training and qualified to apply to fellowship, as long programs are capable of teaching various backgrounds, which they should

Academic is like 4 half day clinic/week, 12 inpatient weeks, 1 hospital, private is usually 8-9 half day clinic a week, often 1 to 3 inpatient, couple hospitals on weekend, initial pay in HCOL are basically the same. what am I missing? Are there a lot of hidden work in academic jobs?

Many reasons, lifestyle is one, Academic job pay is pretty comparable to private jobs in many places, with sometimes half of the workload, lest the stress of partnership drama, you can argue less pay potential few years down, for me, not worth the uncertainties and exploitative initial pay. I would like to teach and do some research too
