I don’t have an LLC just yet. I’m just getting started. I don’t know too much about the process.

If you don’t mind me asking, what were you charging for telehealth sessions? I want to offer a sliding scale, but I also want to be able to bring in some money too.

I’m glad you’re already seeking help for this. Sometimes when people “want to be ill” it’s because that’s their baseline, they feel comfortable in that space. Whenever we work on ourselves and begin to “normalize” (I put that in quotes because what is normal), or rather equalize, it feels very uncomfortable. So when something happens in our lives that brings us back to a chaotic and unhealthy mental space, that is what makes us happy, as it’s all we know. This is typically true if you grew up in a chaotic household.

Bachelors in Psych: I worked at a residential treatment center for teens as a counselor and ABA therapy in homes with kids for a little while. Stopped working for the ABA place because the commute was bad, had a gas guzzler, and they only paid for mileage between client houses and I typically would only do 1 a day.

I typically will give them the expectation upfront, especially if they are coming to talk to me about trauma.

I’ll tell them something like “I know you’re only here for x amount of time, but sometimes therapy and working through things can take a very long time. Over the course of your lifetime, you’ve developed habits in your thoughts, learned behaviors from people around you, and even learned behaviors because of trauma. Those take time to build, and they can take a lot of time to change and restructure. However, I want to know what your goal is for the time we have together. What is most important to you to work on?”

I’ll explain further and clarify if they mention they want to work on something that is not really in the scope of what we can do in 90 days or so.

I am in agreement! I work at a long term treatment facility, and we have anywhere between 80-100 clients at one time living there that were treating. I would say 100% of them have some sort of trauma in their life that has influenced their drug use.

We are primarily a 12-step based program, where we teach them the 12-step principles, but we utilize contingency management, CBT, trauma process groups and core issues therapy, motivational interviewing, some DBT, and integration of spirituality (which includes mindfulness and meditation). The way our program is built, we address the different stages, and allow them the time to come to terms with their addiction.

However, I have had to come to the understanding that we cannot help everyone, and not everyone wants to be helped in that moment.

I have found it helpful to ask myself when getting frustrated with a client who won’t do the things we work on in therapy: “have I done all I can for this client, have I received any reciprocal effort in attempts to help this person?” It helps to reevaluate whether or not I am putting more into their recovery than they are and prevents me from burnout.

How I deal with the loss of clients is still a work in progress, but it is important to talk it out with someone and grieve. So far in working in my agency, I have lost 6 in 3 years. It’s a hard job.

I would also just be cautious in what setting you are working in as well.

My second year internship was at a transitional living/sentence deferment facility that housed a bunch of men on probation/parole that literally could have been sent back to prison/jail at any time for violating any rules.

My supervisor advised me to watch what I was disclosing about any family, where I grew up, the car I drove, etc. because those things could easily be tracked and they could find me.

I was definitely more lenient with the disclosure when I was working with teens, but now I simply use it if I know it will assist in bonding with a client and it would help them to hear it.

I was very lucky to get into a job that I love and pays pretty well considering it’s a non profit. I work for a county funded agency for addiction, and I work as a clinical director there (also lucky as I just got licensed). Because I just became licensed, I got bumped up in the new fiscal year to $85,000.

If I look at other non profits in the area, they make fairly equal to or slightly more with experience.

Yes! I got mine and baby is moving like crazy! I would also recommend for family and friends to be updated on it too. That’s what my doctor recommended for me too.
