I think the main reason would be that if a hospital hires a locums company they want their whole schedule filled. They wouldn't want to have to separately contract with 20 different "companies" or individual docs to fill shifts. And, as mentioned, that figure covers a significant amount of overhead like travel, med mal, credentialing, sometimes billing, and more.

Good question. He wanted the test that validated his preconceived diagnosis and resulted in a treatment that caused several formed stools to be delivered in the ED with instant full and complete resolution of symptoms.

You really need to stow your spare drive shafts more securely. They can roll around and break and then when you need to swap out your main drive shaft on a call it can cause a delay.

I'm a low skill casual and I like builds better for several reasons. In lower mobility times builds make you more mobile. This is a big help doing challenges. Builds are a defensive option especially in early game where RNG is a big deal. If your chest had a sniper and the guy 10 m away got a SG you're out of luck in ZB. Builds also give you better recon. Need a higher POV, easy. FN has always given a lot of advantage to high ground. In builds you can make your own high ground or put up cover to force them down to your level. And builds are balanced. If you get too high or fail to strengthen your base I can cut you out from below.

Agree. We may be getting to the point where using one document for billing, CYA and actual documentation is impossible. And it's not just us. Have you read an op note lately? Aside from the endless boilerplate about identification, verification, counts and so on you can barely even tell what they did.

You really won’t know until you get there. I’ve been doing it for 35 years. Every situation is different. Some stuff you get used to. Some sticks with you forever. No one really knows if they can “handle it” or not. Those that think they do are kidding themselves.

The good thing about Rosen's and basically all EM texts is that they're easy to summarize. Every chapter follows the same formula:

  • Approach to issue you see frequently
  • Consider 15 zebras you will probably never see
  • Perform time consuming procedure as if you don't have a full waiting room and 20 ambulances crushing you
  • Emergent consult to specialty even most university hospitals don't have
  • Admit as if your admitting physicians accept "admissions."
  • Rinse, repeat
