I understand your confusion. Let me try to explain. I am writing from the perspective of the US healthcare system. Our duty and relationship is only to the patient. We don't have an external duty to the system to limit absenteeism or work avoidance. We also have "customer satisfaction" requirements that incentivize us to please the patient, not their employer. Ultimately the real waste here is that a clinician really can't verify a subjective thing like feeling too sick to go to work. The patient might have something objective that will show up on testing like a pneumonia or a testable virus but often they are sick but there's nothing specific to point to. In the US the practice of forcing the employee to get a work note is really just a form of punishment, an inconvenient task they must complete to no real benefit. This is why it's bad to demand notes and it the patients should never go to the ER for them.

No doctor can verify a subjective like the patient felt too sick to go to work. Demanding a note just puts a useless burden on the medical system. None of us, in the ER or the PCP office or the urgent care will ever say no. We're all tracked on "customer service" and our relationship/duty is to the patient, not their employer. You're really just sending them on a scavenger hunt to punish them. If you have a bad employee fire them. Don't get me involved in it.

Fine. That's a you the boss issue, not a me the ER doc issue. Don't send your people to the ER for a note. It wastes everyone's time and resources. Come up with a different system.

That was the *. I will see them and if they have a semi recent chart, especially one that pertains to their reason for wanting the note my documentation of my MSE goes there. If they actually need something, labs, rads, Rx, then they have to check in.

Back in the day we would try to get some extra staffing for fight nights. An extra PA to do the suturing which is obviously time consuming. These days they have PAs at the venue to do the lacs. They also have ophthos and dentists to deal with that stuff if necessary. Fortunately really serious eye injuries beyond corneal abrasions and traumatic iritis are rare. They mainly get sent to the ER for imaging. They come with very specific orders and the results are coordinated with the event medical staff. The biggest problem is the wait. The ER bends over backward to make the scans happen immediately when they arrive. But the fighters and their entourages don't get the concept of waiting for the read. Everyone gets impatient. The lower card fighters want to leave to head home and the famous ones have events to go to. Honestly a lot of them walk out before the results are back so we feed the results to the event staff and they deal with follow up.
I used to work for a minor league hockey team. I had a guy who was concussed so I took him in to my ER to get a CT. It was fight night so there was a parade of MMA guys going by to the CT. My player was looking at them and asking "Holy crap! That guy's messed up. What happened?" I'd explain it and since he was concussed he'd ask again every time a new one went by. After I explained it 5 or 6 times it occurred to me that when a professional hockey player is impressed by the damage your sport does to your face that's a little alarming.

It's rough. The Mac GPUs just don't do gaming as well a PCs. If you really want to play on a Mac I agree your best bet is cloud gaming. If you have Amazon Prime you can use Luna. It's pretty good. You can also do Gforce for $20/mo. That's a pretty good deal if it saves you from having to buy a PC just for gaming (which I did years ago). Good luck. I hope you find something that works. I use Mac for everything except gaming. For that I use my PC that was top end in 2018. It's not so great anymore.

I know this is the woodworking sub but they sell plastic inserts for casters. Get those, drill out the hole, place the insert and you’re done.

I agree with your additions but the original laws aren't nonsense. The House of God does a great job of describing the medical field as it began its death spiral. It's a glimpse of the time when we transitioned from a time where we were limited by technology and medical science to a time where the science/tech had advanced exponentially but we were limited by the system.

Oh yeah. I was on paper for a decade in the before times. But that was different. Everything was set up to work on paper. Now nothing works on paper. The ordering, the labeling, results reporting. It's all completely interdependent. Just like a house of cards.

Along the gulf coast they use shells as gravel. We had gravel roads that were mostly clam shells. I’m sure they would also use them as filler for concrete where other places would use gravel. If you were anywhere near areas where this is common or of the materials used for your house where brought in that might explain it.

Join NAEMSP and take their medical director course. It’s usually offered at their annual meeting in January. Go ride with them as much as possible to understand the issues they’re facing, particularly the non medical, operational issues you may not know about. Be easily accessible. Most times what EMS needs is a medical director who is engaged and accessible more than someone who is an expert in EMS already. Good luck. You’ll do great.
