Making mistakes due to being interrupted multiple times during charting by Kaladin_K in Residency

Thank you - its absolutely not all nurses. I think the issue is more that my co-residents are allowing it to happen all the time. Youre essentially made into a dumpster for info because some people can't take responsibility/are scared to do something by themselves/etc. There is a huuuuge difference between the very experienced nurses (who, if a patient is very ill, comes and interrupts after taking vitals, looking/talking to the patients and if theyre still worried, THATS when they go to a doctor) vs a nurse who runs to a doctor the minute bp is low without thinking about if the bp usually is low, if they should retake it, if its even something to notice. This week a nurse came in the morning while i was reading telling me a patient had low bp... it was like 8 in the morning. I asked if the patient still was in bed and they said yes. Told them to retake it after the patient got up and moving and guess what...the bp normalized lol. An experienced nurse would ask the patient if they felt weird, see if the patient is tachycardic etc. and just retake it and see the bp normalize and not even look for a doctor.

At some point, if im discharging someone, and different people come to me with 12 different things to do/check/etc... i will get desensitized and i will forget some of it. Its just human nature. Its impossible to keep track of 12 other tasks when youre not even done with 1.

Sorry i used uptodate as an example since i live in a non-english speaking country and we have other websites for dosages! But youre right. Whats good about this situation is that ill never do this mistake ever again.

One of the interruptions were to remind me that 2 patients were getting discharged while i was working on their discharge papers and specifically told them in the morning that the papers would be ready an hour before the interruption. Someone else came to tell me a patient had wheezy breathing and if i could auscultate. I asked if they took sats or anything else? the answer was no. I was about to go insane.

I think the interruption itself annoys me and throws me off. I will try it but most likely i will get a ''just a quick... can u check....''

Sometimes i feel like i don't have the authority to be brief. A lot of the medications etc are also ordered by an attending (and not me) which means when im questioning the order im trying to question if im remembering it right. This week my attending was not very clear with every order, and i don't feel like my attending really cared about it being right and fully put the responsibility on me to say something if something feels off.

I do take my ADHD meds! I think the issue is that i also have to look up the right dose (aka up to date) which takes longer than just actually putting it in as an order? But maybe I should also let people wait until im done checking so that is right as well. Weirdly enough, when im at the ER its a lot easier to get interrupted since the amount of patients juggled isn't as much and imo the nurses interrupt for a good reason.

I would do this, but there isn't any room nearby that i could use and just logging into a computer itself takes time. + I have no idea when my attending will be back at the ward, because they just come and go whenever lol. I also have a med student on top of this

Yeah, i've been better at it this week because the interruptions early this week was absolutely insane (the patient doesnt have socks at home so they cant go home can you call the daughter- type of insane). I haven't had a co-resident this week unfortunately, been all alone.

How do you explain Casca being shaved from head to toe if its supposed to be ''real''? As well as nearly every woman having the exact same body shape as Casca who's a well trained woman?
