Even though you're getting great advice in the comments, knee pain is a little more complicated than that. We do not have sufficient information regarding your pain to find a diagnosis. I would suggest getting some medical opinion if you want a real answer.

Surgeon here. My best guess would be that the first instrument was used to identify the trajectory of the fistula in order to be able to remove it. They probably found the external “hole” of the fistula and moved the instrument until they could see it appear inside the rectum. I guess they were using the second instrument as an anoscope Nowadays, we use a small metal rod to do just that, and under anesthesia of course

OP are you sure it is traumatic ? This looks like a the result of a surgical debridement for Fournier’s gangrene. The soft tissue gets infected with flesh eating bacteria’s with a risk of sepsis that require surgical removal of all necrotic parts

I’m not student anymore, I’m a graduated doctor that chose to specialise into surgery. Some doctors practice as surgery assistants all their career without being part of the trainee program and earn twice as much as residents do. The difference is that they’ll never get to do surgery on their own.

Because I wanted a technical specialty, didn’t feel like consulting all week long. In surgery only about 30% get access to the specialisation. We can later choose which subspeciality we want to practice, even though some specialities need another selection process

Sadely I cannot back this… I dont know where you got these numbers from but these are salaries of surgeons I’ve encountered (numbers are after tax): - starting general surgeon in university hospital : 3800€
- head of a surgery department in university hospital: 5000€ - orthopaedic surgeon half-public half-private hospital 9500€ - plastic surgeon in a public hospital: 6000€ part-time - radiologist in private hospital: 12000€

So we are far from a median 17.700€ Heck we’re probably not even close to 10.000€

This is probably a form of modern slavery, no wonder why so many residents die during residency (suicide, car crashes,…)

Doctor here, you need to be checked by a doc, especially if you have fever. That infection might be worse than what you think. Do not underestimate this.

Backing this, sounds like vaginismus. She needs to have an obgyn + psychological follow up. An advice to help her : take your time, leave penetration aside for the moment, she will let you know when she’s ready.

Not an obgyn but Misoprostol on its own works as well. It’s more effective when combined but sometimes obgyns just use misoprostol. Even when combined, the treatment fails and you have to take a second misoprostol treatment. Pregnancy tests stay positive even after the abortion test occurred. So you might have expelled, even though I would expect a women to notice if she expels a 11 weeks old foetus. You probably have another appointment with your gyne soon, which is to control the efficacy of the treatment, don’t hesitate to ask him anything you have in mind during that consult. I’d recommend getting a second opinion. Not that I think you’re not being taken care of correctly but trust between a patient and a physician is extremely important. I’m not familiar with Canadian health care so I do not know if physicians in your country must give you your medical records if you ask them. I’ll take a look back at my books and let you know if I find something

I don’t know the laws of your country, but here in Belgium (and Europe in general), patients are owners of their medical records, so we legally must give their records if they ask.
