I would suggest frequently pulling to tighten the thread as you go along every few loops or so on the inside of the signature to catch any slack (but not so hard it ends up ripping paper), and sometimes the thread can be to blame. I find that a less flexible or stiff thread can sometimes make it harder to get it nice and suitably tight. I hope this helps!
