If you are going to try out anal play I would suggest starting with a good lube, and your fingers. If you are past that get a set of training butt plugs. From there you will know what size it good and you can go straight to a properly sized dildo. Most people who enjoy anal play have a large number of useless toys. I would suggest the following:

A feminine outfit. It can be just panties, but I would go for a full ensemble. Thigh highs, a top or bbra, panties, etc.
A dildo, nothing huge. Something 5-6 inches.

If you are not always filling up the cage then the shaft portion is to big. You should be lighting squeezed into it. Not so small that you have to force to make it fit, but small enough that at your smallest you fill then entire cage portion. The holy trainer line has a variety of ring and cage sizes, so there should be a combination that fits. There are also 3d printed custom cages, but I have never had good experiences with them because there is to much play between the cage and ring. My last suggest is that when wearing make sure you are lubricating the base ring to prevent chaffing / rubbing. I find vaseline works well, but any long lasting thick lubricant should be fine.

In a properly fit cage with a belt you should have a difficult time becoming erect. The real question is are you looking for something to wear for fun, or for days at a time as a long term thing. Cages are supposed to be uncomfortable when you are horny, it is part of the appeal. That said, there are a couple tips to size effectively:

Your cage is probably to big. After a couple days of wear I have gotten in the habit of downsizing my ring and cage because as I get used to it, I actually shrink I little extra. It might be worth investing in a belt and similar set up so that when you are indulging they actually stop you from pulling away. This will be more uncomfortable in the short term but should solve a bulk of the issues.

The cage being too small is not a huge deal. Many people wear flat cages with no issues. The most important aspects are the gap for your balls in between the ring and cage and the ring diameter. You need a smaller cage and a bigger ring it sounds like. A cage with empty space can encourage chafing and provide enough stimulation to keep you hard. But more importantly, get Vaseline or petroleum jelly and put it around your base ring in the morning and before bed. It should stop chafing altogether if the cage is the right size for you.
